I give Clean Detroit and their legal representatives and assigns, the right and permission to publish, without my charge, photographs or videos taken of my image.

I do hereby grant permissions to Clean Detroit, its agents and others working with or under its authority, full and free use of video, photographs and audio containing my images, likenesses and voice. I understand these images and sounds may be used for promotional publications, news, research and/or educational purposes.

I do further certify that I am of legal age and possess full legal capacity to execute the foregoing authorization and release form.

I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

I certify that I am physically fit, have sufficiently prepared or trained for participation in this activity, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity.

I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity.

I understand that, at my own discretion, I will not be asked to or held responsible for attending, exploring, surveying, participating at or otherwise being at locations that I deem toxic, dangerous or a liability to my health or personal beliefs.

I understand that, at my own discretion, I will not be asked to or held responsible for cleaning, picking up or otherwise handling anything I deem toxic, dangerous or a liability to my health or personal beliefs.

I acknowledge that this Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity.

IN CONSIDERATION of being permitted to observe or participate in any way in any Clean Detroit event or activities (“Events”), I, the undersigned, my personal representatives, executors, heirs, next of kin, successors, and assigns agree as follows:

1. To release, waive and forever discharge Clean Detroit and all promoters, participants, sanctioning organizations, facility owners, property owners, officials, sponsors, advertisers, owners and lessees of premises used to conduct the Events, premises and event inspectors, surveyors, underwriters, consultants and others who give recommendations, directions or instructions regarding the premise of the Events and for each of them, their directors, officers, faculty, staff, volunteers, agents and employees, all for the purposes herein referred to as “Releasees,” from all liability for any and all loss or damage, any and all property theft, and any claim or demands therefor on account of injury to the person or property or resulting in the death of the undersigned arising out of or related to the Events, whether caused by the negligence or otherwise.
2. To assume full responsibility for any risk or bodily injury, death or property damage arising out of or related to Events whether caused by negligence of Releasees or otherwise.
3. That this Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement extends to all acts of negligence by the Releasees, including negligent rescue operations and is intended to be as broad and inclusive as is permitted by the laws of the Province or State in which the Events are conducted and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

I understand that my participation in these Events is my choice and I can choose to withdraw from any Event at any time.

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.

I hereby certify that I am the adult parent or legal guardian of a minor child under the age of eighteen years, and I consent to their participation in any Events.

I understand and acknowledge that I am fully aware of and assume all the risks (including but not limited to the risk of serious bodily injury, property loss or damage) of said minor child’s participation in Events. I recognize my responsibility to ensure that said minor child participates only in those activities for which they have the required skills, qualifications, training and physical conditioning.

Knowing the risks described above, I agree, personally and on behalf of the minor child named above, to assume all the risks and responsibilities surrounding my minor child’s participation in Events. To the fullest extent allowed by law, I hold harmless and agree to Clean Detroit and all promoters, participants, sanctioning organizations, facility owners, property owners, officials, sponsors, advertisers, owners and lessees of premises used to conduct the Events, premises and event inspectors, surveyors, underwriters, consultants and others who give recommendations, directions or instructions regarding the premise of the Events and for each of them, their directors, officers, faculty, staff, volunteers, agents and employees, all for the purposes herein referred to as “Releasees,” from and against any present or future claim, cause of action, loss or liability for injury to person or property, which said minor child may suffer or for which said minor child may be liable to any other person, related to said minor child’s participation in Events, resulting from any cause whatsoever, and regardless of fault.
